  Coordinated follow-up to alpha6 for lower-level orchestrators. Adds an at-most-once on_subscribed hook to Client#stream_events so custom turn pipelines submit prompt_async only after server.connected, without replay on reconnect.   Fixes prompt-before-subscribe by waiting for server.connected before the first prompt. Reconnects remain at-most-once.
