  Closes the Rails orchestration gap found by the Travelwolf alpha6 canary. Opencode::Turn now submits prompt_async only from the transport's at-most-once on_subscribed callback after server.connected. Reconnects never replay the prompt, and pre-turn subscription or prompt failures fail closed instead of recovering stale exchange text.\n\nDepends exactly on opencode-ruby 0.0.1.alpha7.\nSHA-256: 5d61100249fac371c952da3b333720ded7bd949c5d422bc96c7f086e1fdf36e0\n\nVerified on Ruby 3.2, 3.3, and 3.4: 55 tests, 145 assertions.
